If you are not happy with the service you have recieved, in the first instance please inform the service by either emailing email@example.com or telephoning 01522 697282. A member of staff will be happy to hear your concerns and attempt to resolve them to your satisfaction.
If we have not been able to resolve your concern you are encouraged to make a complaint which will be handled initially by the service manager and then by the Lincolnshire Community Health Services Complaints Team. You can make a complaint by phoning the complaints team on 01522 309752.
