You Said - We Did: Referral Forms

We have recently made some changes to our referral form when it was unclear where a visit was going to take place, whether it should be at home, a day service, hospital etc. We have always assumed home as this is where 90% of our visits take place, however we have learnt not to assume and on feedback have added a box asking you where you would like the appointment to take place.
